I-693a Vaccination? or I-693 Medical HELP!

Timeline

Okay..Dan just got his visa. His medical was in June. We don't have a copy of this. He does however have a copy of his immunizations. Question-How do we get form I-693a filled out to send in with his AOS? Do we have to find a civil surgeon to do this here in Cullman? What do we actually need for this?

Yes, locate one close to you, call their office and check fees. My wife received all her vacinations and a completed I-693a form sealed in an envelope which we included with our paperwork at the AOS interview. Be sure to bring your xray as some doctors require to see this before signing off on your paperwork.

Once here in the US he would take his vaccination record to a Civil Surgeon and have it transcribed onto the I-693a... they will put it into a sealed envelope and give it to him.... you then send this with your AOS packet to Chicago....

Call around for prices for Civil Surgeons in your area as their prices vary..... he does not need to have another full medical...

Now, people are saying that when they had their medical done for the k1 visa they received a form which they keep. Rose was given a sealed envelope and it was presented to the officer at interview. The envelope was opened, but none of the contents were returned, no DS3025, 1-693 or anything. Only thing she has is one x-ray.

So then how do one go about acquiring a I-693A 0r I-693 when there is nothing?

furthermore time is running out, and likely will submit the AOSpacket minus the I-693 forms.

Print this out if the DCS says you need a full medical!

"C. Medical Examinations

According to 8 CFR 245.5, aliens seeking to adjust status are required to undergo a medical examination performed by a designated civil surgeon to determine whether they are inadmissible under section 212(a)(1)(A) of the Act. To date, applicants for K nonimmigrant visas have been required to obtain a medical examination abroad pursuant to Department of State regulations at 22 CFR 41.81 prior to entry, and the medical examination is not repeated if they apply for adjustment of status within 1 year of the date the examination was performed. They are, however, required to submit with the adjustment of status application a vaccination assessment completed by a designated civil surgeon in order to establish their compliance with the vaccination requirements under section 212(a)(1)(A)(ii) of the Act.

The Service will continue this same policy for the K-3/K-4 nonimmigrants. K-3/K-4 nonimmigrants who file their adjustment of status application within 1 year from the date of the medical examination overseas will not have to submit an additional medical examination. However, the Service notes that applicants whose medical examinations overseas revealed a “Class A” or “Class B” condition (as defined by 42 CFR 34.2( B ) ) must establish upon application for adjustment of status that they complied with those cond itions imposed on the initial admission. Failure to comply with those conditions means that a new medical examination will be required.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

I just did my medical examination last week.

Shop around to check prices from various civil surgeons in your area. Make sure you bring any records you may have. I was in that office for less than 15 min. He drow some blood and gave me a tb skin test (which is possitif.. always have been due to tb immunization when i was a child) did not get any immunization mine were up to date. Had some xray take a a local hospital. pick up my seal envel. conatining the I 693. pretty easy and I sepnd $ 150 at the doctor office and $ 50 at the hospital. the xray were covers by my medical insurance.
