"IMBRA imposes limitations on the number of petitions a petitioner for a K nonimmigrant visa for an alien fiancé(e) (K-1) may file or have approved without seeking a waiver of the application of those limitations. If the petitioner has filed two or more K-1 visa petitions at any time in the past, or previously had a K-1 visa petition approved within two years prior to the filing of the current petition, the petitioner must request a waiver. These limitations do not apply to petitioners for a K nonimmigrant visa for an alien spouse (K-3)."

I've put together the forms for another K-1 filing, but I don't see a "waiver" form that would apply to a second K-1 application within 2 years. Would someone please point me to the correct form?

There isn't a form. Write a letter explaining why you are filing a second fiance petition and ask that they waive the limitation.

Is it correct to include the letter with the application for the new visa?

Yes. If you scroll up to my post from a few days ago, you'll see I wrote...

You will also need to include a waiver request to satisfy IMBRA. Just write a letter explaining why you are filing again and asking for a waiver and include it with your new petition when you are ready to file. There are some examples here in similar recent thread. Check out the IMBRA forum.
