We got the I-693A supplement for my husband yesterday... finally! So now I'm putting the AOS package together. The civil surgeon put the signed I-693a in an envelope, sealed it and stamped it with his medical stamp (as he would a medical exam, even though we didnt have one). Can I open this to include it in the AOS package, or should I leave it sealed?

I'm not sure, just cause I know medical exams shouldn't be opened, but should the I-693a vaccination supplement?

I'd leave it sealed. See if you can include a cover letter for the AOS listing all of the contents and include in there:

I-693A Vaccination Supplement Sealed in Envelope

...to let them know what it is.

But what if they loose it, and do an RFE and I wont even have a copy?

If that happens, I guess you would have to go and get a new one! :) But definitely best to leave it sealed, I think... the civil surgeons seal the envelopes for a reason... the CS who did mine specifically told me not to open it because it had to go 'to the INS' in a sealed envelope. Sounds like most people (perhaps everyone) are given their vaccination supplements in sealed envelopes...

2007 - K-1 issued, moved to US, completed AOS (a busy year, immigration-wise)

2009 - Conditions lifted

2010 - Will be naturalising. Buh-bye, USCIS! smile.png

my doc gave it to us sealed + an addtl copy not sealed to keep for our records.

So did mine... but then, I think it was the same one you used! (Sadly, not the same price, though... I think they just went for the 'think of a number' technique when deciding how much to charge me...) ;)

I fought hard to get that fee, as when I called it was what they said. When I showed up he tried saying $40. What did you pay?

$40... couldn't be bothered arguing it, as I had what I needed and know from reading posts on here that others have had to pay much more!

I know... wish I'd known about that doctor when I got mine done - now I suggest that one to everyone who's looking for a civil surgeon! :)

They were really good about it. They let me fax it to them and they let me send a check after I got it back in the mail. Only took 3 days. :)

Thanks for that, Gary! I am going to call them later, was hoping they'd let me fax it. I would be willing to give them a credit card if they wanted the $10 right then. :D

Oddly enough they don't do plastic at all. Just check and M.O. .
