Don't worry about the vax card.  As of now US still isn't checking them at all, and if they start I am sure US would accept a LGU card with a minor misspelling.  You will need a negative COVID test within 3 days of departure.  For US a rapid antigen swab test is fastest, easiest and cheapest.  CFO is still a thing and would probably be the biggest pre departure hassle.

thank you so much, I'm still kind of overwhelmed with things I have to do before my flight, can you help me out with a list of things I should have on me to present at the airport? it would be such a big help to me.

Regardless of the airline you've booked, the PAL website is a good source of info for the pre-departure requirements at the airport -- https://www.philippineairlines.com/en/covid-19/departingfromtheph

As others have mentioned, in addition to the regular requirements, CFO certificate is required for those flying with immigrant visa.

Hello same po tayo kasi gusto ko sana magpa schedule sa "yellow card" when i saw their website Dec pa may available. Same question if needed ba talaga siya kasi walang slot.

hi just wanted to get back to this post, I've attended PDOS this morning and regarding yellow card the CFO personnel clarified to us that it is not mandatory to have, only for OFWS . But I am still a little confused since I have been seeing so many yt videos of travelers that have it in possession and are saying that it is needed

hello, i made an appointment to get the yellowcard as a formal document that iam fully vaccinated kasi yong vaccination card from the LGU Is not computerize and my name and other info their was all handwritten, hindi masyadong klaro ang pagkakasulat kasi maliit lang ang spaces nung card and no offense it looks informal lang siya sa akin. In wednesday na yong appointment ko po.

hindi po siya expedited, pwede siyang ma expedited kung last minute na you need to travel abroad mag email lang ng flight documents yon ang sabi sa fb page ng BoQ ng Cebu. Nong unang attempt ko to schedule an appointment is Cebu main office lang ang nakalagay pero Dec 1st week lang ang pinaka available, however nong nag post ang BoQ ng Cebu nga after 3hrs na merong satellite office in Robinsons Galleria-Cebu don na ako nag attempt 2nd time and nakakuha ako ng last week of Sept and meron silang slot sa whole month ng Oct. And sa vaxx cert kumuha din ako kasi mag aask ang online form ng yellowcard sa vaxx cert sa front and back pic as their requirement ng and wala pong bayad ang cert pero sa yellowcard is 300.00. Ang requirements kasi na e upload to get appointment ng BOQ is ID, PASS ID, VAXX CERT

im glad you were able to get your ICV! in my case naman available slots is by January na eh but I can't wait for that na, sabi naman ng pdos di mandatory yun for us but I think its better na kuha ka nalang para meron. So I think ill just book a flight na without it

PAL has filed for bankruptcy in USA, would be funny if they folded up.  USA shouldn't let them file in the USA, I am sure American Airlines can't file for bankruptcy in Philippines.

Don't worry about the BOQ Yellow Card for US.  It is not required for US.   There 'might' be some other destinations that require the BOQ Yellow Card.  OFW's in Hong Kong have been mentioned about that.  In the future there might be some other countries that might require it since it can be authenticated overseas.  But US does not require it now and probably would accept the LGU card if they start requiring it in the future.  So if it's easy to get.  It may be worth it for peace of mind.  If it is dificult or impossible to get, don't worry about it.  It's not important or required for US travel.
