Incorrect Middle Name

Hi, all. I would like to humbly ask for any advice you can give in our current situation.

 

My grandma filed the petition for my dad including us. We already received a welcome letter from NVC and now getting to prepare and process any necessary action moving forward. I am the one consolidating required documents. I am also helping my grandma to fill out the I-864 form (she's already 86). This is where the issue arise. My grandma's middle initial in the filed petition form before is incorrect. So her name in NVC has incorrect middle initial. She mistakenly used her first husband's last name as her middle name and her 2nd husband's last name as her last name. Since her 1st husband died, she should be using her maiden name together with her 2nd husband's last name. All her documents (passport, tax, certification, etc) is showing her maiden name with 2nd husband's last name, which is the correct one. 

 

Please let us know any necessary action to fix this to avoid any issue when we got in the interview stage as this might be the reason to get our visa denied, due to inconsistency of petitioner's name.

Hi, Timona! Thanks for replying to my concern. Just a quick follow up question if you dont mind. What name should I put in the I-864 form of my grandmother, the name she used when she filed the I-130 form before (the incorrect one) or the name that she currently uses in her passport and other documents?
