what kind of form i need to prove that i am in my husband insurance?

The proof I had gotten was a letter stating all the member on the insurance plan from start date to present. A copy of the card will not help if it doesnt have a start or expiry date on it.

I do not believe you would get this information from the website. I suppose you can call customer service and make that request or you can use the insurance tax document that you normally get yearly. I have BCBS and I called and got the letter that very same day. I hope this helps.

Hi! They sent me the letter but it says Jan 2021 to present. Do you think this is good?

Yes, but they gave me from 2021 one:(

Please tell them you need the entire history for immigration purposes. I would assume that would be okay as long as you have insurance.
