RE: I-765, Application for Employment Authorization

Applicant:

A-Number:

To Whom It May Concern:

My Adjustment of Status case, MSC....., is pending. I’m filing my Advance Parole Document (I-131) application concurrently with this request for an Employment Authorization Document (I-765).  I filed the I-485 after July 29th 2007, and paid a one-time filing fee. Therefore, as an Adjustment of Status applicant, I am not required to pay additional USCIS fees for this I-765 request.

Enclosed please find form I-765, Application for Employment Authorization, and supporting documents as follows:

  1. (2) 2”X 2” size colored photos with name and A# at the back

  2. Form G-1145 - E Notification of Application/Petition Acceptance

  3. Copy of I-797C  Notice of Action, receipt notice of pending I-485

  4. Copy of I-797  Notice of Action, approval notice of current I-765

  5. I-765 Form -  Application for Employment Authorization

  6. Copy of my current Employment Authorization Document (I-512 serves as AP)

  7. Copy of biographic page of passport

  8. Copy of K1 visa page and US Entry stamp

  9. Form I-94 Arrival / Departure record

  10. Copy of marriage certificate

Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be requested to submit original documents to an Immigration Officer if requested

Sincerely,

""

Looks like you had everything.  We added the state ID (if you have one).

Also, are you using USPS or UPS/FedEx/DHL?  Depending on which company you are using, it will change where you mail it.
