I just finished filling out Luz and Robins AOS forms. I read the guides and the instructions on the AOS forms. Could you please double check me please and let me know if I left anything out?

For Luz:

1. I-485 form

2. Birth certificate for Luzviminda (Tila) Cornelius

3. Copy of Luzviminda (Tila) Cornelius’s passport including Visa page and front page.

4. 2 photographs

5. I-693a vaccination supplement filled out by a civil surgeon.

6. G-325A Biographic Information Sheet.

7. Copy of I-94

8. I-864 Affidavit of Support form

9. Copy of I-129F approval notice

10. Copy of Gary Cornelius’s (petitioner) divorce decree for previous marriage to Carol Cornelius.

11. Copy of Luzviminda (Tila) Cornelius’s marriage contract with annotation of annulment for previous marriage to Ricarte Reyes.

12. Certified copy of marriage contract between Luzviminda Cornelius and Gary Cornelius (petitioner).

13. Check for $395.

For Robin:

Same as above with Luz's BC included.

Looks good to me. You can compare with mine if you want :) I dont know if you received your Notice of action for the sponsor's change od address, but I assume you also filed the forms with your new address like I did ;) I received the NOA's at my new address like planned

Contents include:

-Payment in the amount of $395.00 (I-485 and Biometric services)

-I-485 Application for Adjustment of Status

-Copy of birth certificate + translation

-Copy of K-1 visa + I-94 Arrival/Departure record

-G-325A and 2 passport style photos

-Vaccination supplement I-693a

-Copy of I-129f Petition for Fiancée Approval notice (I-797)

-I-864 Affidavit of support (Sponsor: Josh)

-Copies of Federal individual income tax return and W-2’s for 2004, 2005 and 2006, and letter from sponsor’s employer

-Copy of USPS receipts for sending the forms AR-11 Alien’s Change of Address card and I-865 Sponsor’s Notice of Change of Address

-Copy of I-865 Sponsor’s Change of Address Receipt notice (I-797)

-Payment in the amount of $180.00 (I-765)

-I-765 Application for Employment Authorization

-Copy of I-94 Arrival/Departure record

-2 passport style photos

Copies of your recent Fed tax returns, w2's and all other applicable IRS forms (see I-864 checklist)..

That is included in the I-864 Affidavit of Support form. I had the last years tax forms, W-2's and a letter from my employer included.

I filled out the online form for Luz and Robins change of address. We didn't get any notification of that though. I didn't fill out anything for myself though.

look for the I-865 Sponsor’s Notice of Change of Address

You will receive an Notice of receipt ( I-797) for it. And it is normal that you wont received any notice for the AR-11 for them. Sponsor has 30 days to notice USCIS from change of address.

I filled out the online form for Luz and Robins change of address. We didn't get any notification of that though. I didn't fill out anything for myself though.

I included 3 years worth of tax returns. I read somewhere someone was RFE'ed because they only had 2 last years, and not three.

As for my husband's AOS+EAD packet..

I've included a photocopy of my husband's NBI clearance certificate with our AOS & EAD application.I read that background checks can take a while so I figured they can probbably use the NBI clearance as a "reference". I know that we still need to go through background checks but I guess it won't hurt if I included it (I HOPE). Anything that'll avoid us getting a RFE or can speed up our AOS process a little bit sounds good to me. :)
