First of all thanks everyone for the great forum I learned a lot from this forum. I was ready to file concurrently my I-140 and I-485 suddenly I learned from my attorney that I need to file a form called I-994. Unfortunately, it is very painful and I know it is a new form and we do not have many experience. IF anyone have any experience regarding my questions below, I will appreciate it. Here are my questions:

1) On the income part I list me my wife and my son. We file jointly with my wife (my wife cannot work so she has an ITIN) so we have one tax transcript which is the same for all of us. On the total income fields, do I put the same number for everyone? Or on the parts for my wife and son any income contributed to my wife and son which are basically 0$ each?

2) There is another question asking about an approved I-140 and since I am doing concurrently I do not have an approved I-140. If I click yes and write concurrent, I can skip the whole section but since there is no such ioption, I click no to that then I need to fill education history etc. My highest degree is a Ph.D. from USA but the rest are out of the USA, do I need to have all degrees including high school diploma to be evaluated? I am very time-pressed with this application so want to file asap and on top for high school I have no transcripts whatsoever even the diploma  is currently held hostage at my undergrad institution. If I do not submit credential evaluation will this result in an RFE or rejection/denial? Any experiences from people in similar situation that have already submitted. I read some people not submitting high school diploma (Did you list it there?), did you receive an RFE?

3) I have employer sponsored health insurance and there is no where any info about its termination but I have documentation that it is current as of today.

Thanks
