Hello guys! A week after my interview, US Embassy sent me a DS-5535 form. They asked me to fill this out. They didn't ask for any other documents, also they didn't give back my passport. Based on my research, most applicants who got this had worked overseas and it took them months (others took more than a year) of waiting to get the visa. I never worked abroad. I traveled to Hong Kong two years ago, went there for a tour and stayed 4 days and 3 nights. Anyone else having the same case?

That is a little odd being you were not an OFW or traveled extensively.

Yea it was really odd and I can't think of any reason why embassy required me to fill out this form.

All you can do is complete the form and send it back by 2GO,  include a copy of the email/letter the embassy sent you.
