Getting vaccination supplement without worksheet

I'm wondering if it's possible to get a civil surgeon to transcribe the I-693A/vaccination supplement form without having the vaccination worksheet that was received at the medical? Long story short, I can't find my vaccination worksheet, it was apparently misplaced in all of my paperwork (though I still have the handy X-Ray kicking around), but I do have my vaccination record from the Ontario Ministry of Health. Does anyone know if this is acceptable evidence for the civil surgeon, or will I have to try and get another copy of the worksheet from the Ottawa doctor, if she'd even still have one? Or what are my options?

This is our experience. My husband had his mom send him a copy of his vaccination record from his doctor back home in NS. We in turn took that to a local CS and he transcribed it for us, for a fee of course. I'm not familiar with the Ministry of Health, but if what you have contains your vaccination record as you say why not call a local CS office in your area and ask them. Civil Surgeons should know whether or not they can use what you have. Hope this helps. :)

I agree with the others.

I had my vaccination record from the local health office in London Ontario and I brought this to a civil surgeon. He didn’t have any problem filling out the form with that.

My husband used his vaccination record card from his GP in Jamaica it was updated with his shots at his "medical", he also got a Tetanus shot while in the USA and that was updated to his original vaccination card. So that's all we had to show to get our I-693A filled by the CS.

I had my international vaccination passport with all the stamps, dates and doctor's names who gave the vaccines. Was no problem, they even were happy that people actually possess a record of all their vaccinations in one small booklet.
