Hi all, I have looked at my ds-3025 over and over and I really need your advice on whether I need to get another (partial) medical done in the US by a civil surgeon to complete my vaccination record.

 

Below is part of my form:

My medical was 2 weeks after getting my MMR shot (I couldn't find proof that I had them as a child) so I wouldn't have been able to get a second one administered at the medical anyway, but no waiver has been marked on the form. Is only 1 MMR vaccine required, or should I worry that they didn't mark a waiver and therefore I should book an appointment with a civil surgeon and get another shot done?

Only one dose is required.
