Additional Information (Employment and Address History)

My questions are regarding the Employement and Address History sections for the K-1 Visa.

 

How do I properly include additional information for address and employement history?  On the application itself, it says to "use the space provided in Part 8. Additional Information" but I've also read on this forum that people just add a word document at the end with a list of addresses.  

 

Are there any examples/templates available?

How many addresses and employment you had for the last 5yrs?

I have over 5 for each.

You can use the same format for addresses, but for us.. We use the i129f part 8..since his work is more than his addresses..
