Need help with some questions on form I-485

Hi, I’m getting ready to file form I-765 along with other forms. I’m stuck with some questions, so if anybody can help me, i appreciate it very much.

 

okay here are the questions that I’m stuck with.

 

on part 2 under Information About Your Last Arrival in the United States

 

21.a They asked for my I-94 which I have. So I put that in

21.b They asked for passport number of my most recent issued passport.  I have a new passport that I renewed 3 years go. So I’m guessing that’s what I’ll put in it.

21.c Travel document number. I don’t have any.

21.e expiration date of passport. So are they asking about my current passport? Because the passport where my I-94 is attached expired a long time ago. 

 

And on Form I-485 what is a derivative applicant?
