Civil Surgeon to fill I-693 in Jacksonville, Florida

Hello everyone,
I entered the US with a K1 visa and got married to my fiance'. I now need to apply for Adjustment Of Status and need a civil surgeon to certify the form I-693.

I live in Jacksonville and the ones I called want to do the whole medical from scratch even though the one I did before immigrating is valid for one year.

Any suggestions? Any contacts of a physician in North Florida that only charges for transcribing the vaccinations and the required parts of the  form I-693?

I would suggest to look up on the site:  I-693

I remember, when we filed for AOS, we did not need this due to the fact we filed within one-year of when the medical exam was done in her country. 

Review this here I am positive your answer will be found.

I need it because I never got the vaccination or the chickenpox disease. They did not sell it in the country where I was living (UK).

I just got it today for 150 $ (+ 150 for the second shot) since I don't have a health insurance yet.

I need a physician to certify my vaccinations through the form I-693 without doing again the full examination.
