Form I-693

Really confused here.

 

So I got a letter today from USCIS asking that Form I 693 was not submitted and I will have to send one at a later date when requested.

 

Does this mean I have to do another exam? or just a completed form signed by a doctor?

 

I already had my medical exam done, documents were handed over at POE when I came in May 2017 All I have in my possession are my vaccination papers and the cd.

 

Any thoughts ?

The copy of the DS-3025 that you sent with your AOS application is fine enough as proof at this stage that you already had your medical overseas. They might've lost it though and sent you this letter because of that.

 

You should never send the only copy you have, you should still have the DS-3025 the doctor gave you, or at least a copy of it. Only send copies to USCIS unless they specifically ask for originals.

I just produce this document and I have no need to have this I693 filled out?

 

worse case scenario: if I did indeed not have this DS3025 could I have go to the office where I had my examination for a copy? (and I'm sure it's not for free) ?

question: found some papers but they're my vaccination documentation worksheet. is this enough to produce?

 

I'm not sure exactly what a DS3025 is (my exam was in Canada)

Hi. I filed for my i-485 in January of 2017.  Today (13 months later) we get an RFE stating that we need a complete form i-693 completed by a uscis civil surgeon.  It states that my overseas medical examination expired on 7/20/2017 (exam was done overseas on 7/20/2016).  I didn´t think that vaccination records had an expiration date.  Is it true that I need to take all these tests again?  Thanks in advance.
