After receiving the  NOA2 I am of the understanding that you have to fill the DS-160 form online in order to schedule an interview, however I’m not sure when we are expected to pay for this. Do you pay online or at the embassy?

Your instructions are online, step by step, and will tell you exactly what to do including when to pay. When London receives your file, they will send a letter with a link to this page on their website. You can start preparing now.
