hi everyone, just wondering if someone can give me some advise / confirmation.

I'm very confused about the name information on DS 160 form, it clearly stated that i need to put any name that i have used (others know me of).

Basically my legal name is my Chinese name Zheng Yong Yu, however it's hard for none mandarin speaker to pronounce it,  so i made an English name for myself as Christy.

all my friends / colleagues knows me as Christy, however Christy Yu is not a legal name and was not used in any official documents such as passport / divers license / medical card / bank / credit cards etc.

but according to DS 160 instruction I still need to put Christy Yu (almost like a Nick name) in the form.

I'm worried they will ask me to do a police check based on my legal name as well as Nick name Christy Yu, which I'm sure AFP (Australian Federal Police) won't be able to find relevant records that is related to me under this name Christy Yu.

 

can some one please confirm

 

1. do i need to put Christy Yu in DS 160 form under other name used?

2. Do I need to do a police check on name Christy Yu (my nick name?)

 

i found these instruction on police check, mentioned all aliases previously used name need to be included, but I really don't know if my English name (Nick name) counted as aliases name or not?

can some one share some thoughts and advise on this?

ou must apply for an Australian Federal Police Certificate (AFP). Australian police certificates are valid for 12 months from date of issuance.
Your certificate can be based on a name check or fingerprint check.
If it’s based on a name check, it must contain all aliases previously and presently used. Failure to provide a name check certificate with all aliases may result in the consular officer refusing your application for a police certificate based on a fingerprint check.

Yes,  it is an alias/other name used commonly which is what the form is asking to provide.

Yes you need to list it as an alias on the form, as noted in the instructions you quoted.

thanks, but Australia Federal Police only do police check on legal name, i have nothing to prove my Nick name is used anywhere that is official.
