Well, the question they're getting back to is ties to a foreign country. Sweden is a low risk country so 90 days on ESTA is normal - other countries are more restricted on visits. If the boyfriend doesn't have strong ties to Sweden and intends to stay for a longer period of time (like 90 days), the border officer might think there's some type of intent to stay longer or adjust status. Most interactions with border control aren't that in-depth. They might ask about his ties to go back to Sweden - does he own a house? have kids? a job that he only has 4 weeks of vacation from and then return? etc.

If you are looking for him to come back frequently, you'll need to visit Sweden and take advantage of the 90 days rule for Shengen Visas (90 days of any 6 month period). Otherwise, frequent long visits get ESTA removed and he has to apply for another visa. Alternatively, when/if you are ready, there's K1 and CR1 visas for immigration.
