Name: Mary Hope Kelly   (Hope is her middle Name Kelly is Surname Before Marriage)  Mary Kelly Reyes (married Name)

Once again, what name is on her passport. The name that is on her passport is what dictates which names will go in which location. If you don't answer this question then I can't help you.

The name on her passport Given Name, Middle Name (surname before Marriage) and Surname (married Surname)

                       Mary Kelly Reyes (Married Name with Maiden as Middle Name)

 

So:

Other Given Names Used: Mary Hope (with middle name when unmarried)

That's just an example but YES you got it right.

So for the first part (before you got to the "other names" question) it should have been:

 

Surnames: Reyes

Given Names: Mary Kelly

 

 

 

For the "other names" it will be:

 

Surnames: Kelly

Given Names: Mary Hope

 

 

 

Sorry, wasn't trying to make it difficult, but it's not safe to assume that she had already updated her passport. And the names were getting confusing. Just wanted to make sure I had them straight.

Yes. She just updated her passport recently so there will be no problem.  In the Philippines we only use the first name as the GIVEN NAME, without the middle name.  That's why i wanted to make sure that i'm doing things right.  Thank you.  Thank you very much.

No worries. Sometimes in the US you will see the the "given name" split out as first name and middle name. I rarely use my middle name unless it is a situation like this where they want the full legal name, but here it is still counted as part of your given name since legally it is part of your name.

 

I also wanted to make sure that she didn't just add your surname to hers. Some cultures do that, so then her surname could have been "Kelly Reyes", "Reyes Kelly", "Kelly-Reyes", or "Reyes-Kelly".

Yes, that was why I kept asking about her passport. The first part must match the passport.

No..No.. in our country we only use one surname not combined.  One last thing... how about the Full Name in Native Alphabet.  Searched google and many answered does not apply.  do i have to do that too?  because we also use english alphabet.
