Does this look filled out properly to you?

 

the page number, part number, and item number thing kind of confused us so I'm hoping the way we filled this out makes sense. 

Do you see this as being filled out wrong and causing an issue?

That is how I did mine. Be sure to keep an exact duplicate of your package and good luck to you both.

Thank you! I just realized, what about the top left part of this page, where it says family name, given name, middle name. Is that supposed to be filled out?

Okay thank you, I wasn't sure if that's what they meant, or if it was just a second part to the previous page asking about the "preparer" if using one.
