I am from Canada and will be filing for AOS soon.I haven't yet found or made an appt with a civil surgeon for this,and i'm confused as to what this will actually entail.Also,i've read in various posts people talking about things they had to bring,previous medical checks or records from childhood etc.I don't have anything of the sort,i don't even have my Canadian health card with me (though i could have my parent's mail it to me if they can find it)-if that is even necessary? I don't understand this part of the process,if someone can shed some light..thanks!

05/04/07 -married

06/28/07- AOS/EAD/AP- mailed

06/29/07- package received

07/09/07- checks cashed

07/10/07- NOA1 for I-130

07/12/07- NOA1 for AOS,EAD

07/12/07- biometrics letter received

07/13/07 -rfe payment for I-131 ,wrong amount

07/14/07- touched-485 &130

07/17/07- rfe sent for I-131

07/21/07- rfe for W2s/tax info..guess they lost it

07/31/07- biometrics appt

08/01/07- EAD touched

08/07/07- rfe sent for 485

08/10/07- rfe received,485 processing resumed

08/22/07- receive letter for interview 11/20/07

08/25/07- AP approved and received

09/07/07- EAD card received

10/17/07- received SSN (took about a week after applying)

11/20/07- Interview- more evidence requested

12/14/07- Evidence submitted.....waiting...

12/26/07- Approval notice sent/ Card production ordered

The medical examination is part of the AOS process. If you are in the US, schedule an appointment with a civil surgeon. To locate a civil surgeon in your area, use the civil surgeon locator on the USCIS website: https://egov.immigration.gov/crisgwi/go?act...office_type=CIV

"Shop around" for a civil surgeon since prices vary greatly. Your insurance won't pay for the medical.

The medical examination includes:

1. Physical examination

2. Tuberculin Skin Test

3. Serelogic Test (Blood Test)

4. Vaccinations

People between the ages of 19 and 64 require the following vaccines:

Td

MMR

Varicella

If you don't have written proof of the required vaccinations, the civil surgeon will do a titer test.

You will receive the results in a sealed envelope. Don't open this envelope; submit it with your AOS application.

So - just to elaborate further...

When I got checked out by a Civil Surgeon, she gave me a blood test to test for Syphillis & HIV. She also gave me a Tuberculin skin test. No physical - just a few questions about how I felt, any problems with my health, etc.

She asked me if I had vaccination records to show that I was current for Tetanus and whether I had records for being vaccinated for MMR (Mumps, Measles & Rubella).

I had to get a new Tetanus shot and show health records that I had immunity to MMR. I got these from my regular doctor at Kaiser.

Several days after my appointment with my Civil Surgeon, I returned with all my paperwork and she provided me with a signed and sealed I-693. She was also nice enough to give me a copy.

For my visit to the Civil Surgeon I paid $180 (in Mountain View, a city 1 hour south of San Francisco).
