Copy of a document needed, which form to file, G-884 or G-639?

My wife's I-485 was approved last year; I'm trying to obtain a copy of my company's healthcare insurance enrollment form that was submitted along with other supporting documentation when applying for adjustment of status. Do I use form G-884 or G-639 to request a copy of the form?

Hi, Can you just print  from the website? When I log into my insurance benefits, it lists the members of my household as all individuals who are covered? You can try but when you submitted the documents they warned you that you may or may not receive them back, that is why they asked you to keep copies.

Company's HR didn't process the form and can't find it, that's why I'm trying get a copy from the USCIS to prove that the form existed.
