11.    Copy of the Birth certificate (front and back) for the US Citizen or a copy of ALL pages of the US Citizen's passport issued with a validity of at least 5 years or a copy of the US Citizen's naturalization certificate (front and back). This is used to establish citizenship.

 

Question per the statement above:  Do I submit copies of all pages, including ones without stamps?  I have working annual KITAS for Indonesia, I will include that also.  Also, I live in Indonesia now and do not have a copy of my birth certificate available.

TCO3 - every single page, that's correct! Even the ones with or without stamps. We even copied the front and back covers... perhaps that is a little extreme, but didn't want to take any chances.

02-03-2017: Case Complete 

03-08-2017: Interview and APPROVED!

04-21-2017: POE Philadelphia

We only submitted the front page of the passport. Btw Jakarta Embassy is very easy in term of getting approval for K-1 visa.

my passport is new and only one year old. I still have my old one. Should i include all of those pages also.  Knowing that it needs to go 5 years back?
