DS-3025 without date

Hey everyone!


I might worry too much about this but I can't sit still ever since I discovered that the doctor at my medical left the bottom box blank of my DS-3025 where it asks for the date of the exam next to the doctor's signature. In addition the box where it asks for my birthdate is blank too and where it asks for the exam date on the very top the date is 08-16-1990 (August 16th was the exam but obviously not in 1990.. that is my birthyear). The form is marked as complete. 


My question is do you think USCIS will have a problem with this? It doesn't have my birthdate but an exam date on top with a wrong year...
