I-485 Consulate where visa was issued question.

Getting ready to send my package ASAP, and there is just one more thing I am not sure of (I hope).

On I-485, part 3 I have the following questions:

1. "Place of last entry into the United States" Ok, I went through U.S. customs in Canada, and after reading a few threads here I am to understand that I might put in the city I departed from? Currently I have the first place I touched down at, Dulles, VA. Should I change it to the Canadian city instead?

2. I was pulled aside at customs and briefly spoke to a guy, so that means I was inspected by a U.S. immigration officer right?

3. I don't have a nonimmigrant visa number, so I put in N/A. Under "Consulate where visa was issued" does that pertain to the visa I was granted, or should I just type N/A since I do not have a nonimmigrant visa number.

1. These Canadian airports have US pre-clearance. If you departed from any of these, you cleared US Immigration and Customs prior to landing at IAD. The Canadian city will be your place of entry.

To note on number 2: Anytime you talk to an immigration/customs person before going into the USA, you were inspected. Not being inspected means that you illegally snuck into the USA without going through customs and immigration.
