Medical done and transcription of vaccines

Timeline

Hi guys,

I went for the medical exam today, it was very straight forward and quick and the staff was nice (as they should, considering how much money I had to pay :/).

I have decided to get my vaccinations (the 10 years boost for Tetanus and the flu jab) with my GP, but unfortunately they weren't able to fit me in before the medical and I will get them done next week, the nurse at Knightsbridge gave me a copy of my vaccination record that she said I should take with me to the US, also I know that given that I will get my vaccines done after the medical, I will have to get the documentation to a civil surgeon to get it transcripted along the form I-693.

Now, my question is, what paperwork do I take with me when I go see the civil surgeon?

As per my understanding, it would be the following:

- FULL form I-693

- vaccination record released from Knightsbridge doctors

- vaccination record released from my GP certifying that I did indeed get my shots done after the medical.

Could you please let me know if this is correct?

That's correct. They may also want to see an ID in the form of your passport/visa. The instruction sheet for the I-693 will describe how a K1 should fill it out. It will only be some of the pages that you and the CS will fill out. Our CS gave us a copy of what was placed in the sealed envelope.

Hey,

 

I'm in this exact same boat right now. Where did you go to get your vaccination supplement forms (I-693) completed? The docs in Brooklyn are quoting me around $300 for this service.... :S

Hi! I went to see Dr. Blau at 305 Broadway in Manhattan, the zip code is 10007 and the suite number is 444 on the 4th floor.

He's super nice and he only charged me $60. You'll have to wait about an hour and it's a walk in so you don't need an appointment :)
