This is probably a silly question, but I just want to be sure I am going this correctly.

On the payment page, it asks for a birthday and a name. Is this supposed to be my name and birthday (the cardholder)? Or the name of my Fiancee (the one who is getting the Visa)?

The confusion stems from the fact that the textbox says "Card Holder Name" but I am pretty sure they mean the applicant name, not the card holder.
