Anyone with an RFE for I-693 Vaccination Form? Help!!

Timeline

On 7/15/2016 we got an RFE, it read like this:

Notice to Applicant

An initial review of evidence submitted with your Application to Register Permanent Residence or Adjust Status determined that Form 1-693, Report of Medical Examination and Vaccination Record, has not been submitted or has been submitted and is not sufficient for the following reason(s):
Form 1-693, Page 1. Part 1 is blank. missing or not signed by the 1-485 applicant.
You will be required to submit a valid, completed, and signed Form 1-693 at a later date. Please follow these instructions:
•If you receive an interview notice in the mail, you must bring your updated Form 1-693 to your interview appointment.
•If your case does not require an interview, you will receive a Request for Evidence for Form 1-693 with instructions on where to send Form 1-693.
DO NOT MAIL DOCUMENTS AT THIS TIME. Mailing documents before receiving one of the notices referenced above may result in a delay to your case.

Long story short, when I got my medical exams done the doctor never asked me to sign the form with the results. :ranting:

Basically they did the exams checked my vaccines, charged my credit card, sealed the envelope and handed it over to me under the warning that I could not open said envelope. For my own records, I requested and paid $20 extra for my own copy. When I sent my AOS package I included the sealed envelope and called it a day.

52 days later I got this RFE, I checked my own copy and in fact there was the space on page one where my signature was missing. I didn't want to contact the doctor's office until USCIS gave me further instructions, be it to mail it to me or an interview date, but alas it's been almost two months and nothing from USCIS. On Monday I will go to the doctor's office and show them the hard copy RFE

  • what should I expect?
  • a transcript?
  • what if they want to charge me?
  • has anyone received or dealt with something like this?

ROC - Removal of Conditions (2018-19)

 

12/10/2018 Sent full application for ROC to Dallas Lockbox (Forms: i-751)

 

Day 01 - 12/11/2018 - USCIS received package (priority date) 

Day 08 - 12/19/2018 - Printed  Notice date for NOA (didn't receive on mail until way later though)

Day 09 -  12/20/2018 - Called USCIS gave my alien# and they confirmed the package was received. within the hour I got a text message with my case number

Day 22 - 01/02/2019 - Received NOA Letter on the mail extends GC for 18 months. idk the exact date  I got it on the mail because I didn't write it down right away, this is when I scanned it. Notice date says Dec 19

Day 191 - 6/20/2019 - Called USCIS gave my alien# and asked for update 15 to 19 months  (March 2020 - August 2020) from filing date

Day 14 06/07/2016 We received i-797C for residence (i-485) and petition of relative (i-130 )

Day 20 06/13/2016 We received i-797C for employment app (i-765)

Day 25 06/18/2016 Received Biometrics Appt for July 1st

Day 38 07/01/2016 Biometrics Appt

Day 52 07/15/2016 Request for evidence: Missing Applicant Signature on form i-693 (page 1 of medical exams)

Day 83 08/15/2016 Change online status to "EAD: New Card Is Being Produced"

Day 86 08/18/2016 Change online status to "EAD: Card Was Mailed"

Day 90 08/22/2016 Received EAD card on the mail 

Day 90 08/22/2016 Applied for SSN - card to be mailed in 30 to 90 days

Day 106     09/07/2016 Received SSN card on the mail (took 16 days!!)

Day 182     11/22/2016    called for update, said still within processing time and to give it a month
Day 211    12/21/2016    called for update, said still within processing time and to give it a month
Day 213    12/23/2016    Got USCIS Text. Case updated Online. Interview scheduled for January 31st 2017.

Day 238    01/17/2017   Case updated: Notice Was Returned To USCIS Because The Post Office Could Not Deliver It

Day 238    01/17/2017   Filed E-Request: Service Request for Non-Delivery of other notice

Day 243    01/22/2017    Received NEW COPY of Interview Notice in the Mail for Jan 31st Interview 
Day 252    01/31/2017    Interview Day 

Day 252    01/31/2017    Case updated: Form i-130 and Form i-485 APPROVED!!! 

Day 254    02/02/2017    Change online status to "GC: My Card Was Mailed"

Day 258    02/06/2017    Received Green Card on Mail

My doctor would'nt even touch our i693 vaccinations, just included a blank one. Im curious on what you need to do as well. Im planning to go before a civil surgeon after arrival and get the 693 filled out properly and affix signatures..

Are you going to a different doctor than did your i693? He might charge you a fee to transcribe your vaccinations again onto new form you can sign and submit when necessary. Maybe you could also print out another blank one, you fill it out based on the copy you have, exactly, and just ask him to sign. He might charge but if he is half human might be incidental. I hope anyway, logic doesnt always add up to everyone.

  • February 14 - EAD  & Advanced Parole Approved and being mailed. 
  • May 4 2017. Approved without interview, Card being Produced
  • May 7, 2017 recvd approval for i485
  • May 10 2017 rcvd Green Cards in mail.!!
  • Next step in 20 months to remove conditions.

  • February 5, 2019   - Printing package;
  • February 11, 2019 - ROC package mailed;
  • February 13, 2019 - Package delivered to Dallas Tx;
  • February 15, 2019 - Credit Card Charged;
  • February 19, 2019 - Electronic Notification Rec'd, Case Location LIN - Nebraska;
  • February 21, 2019 - Received NOA with 18 month extension for wife ONLY, not 14yo dependent who is on application;
  • March 12, 2019      - Notified USCIS that we never rcvd i797 for 14 yo minor, they provided a case # and submitted a Svc Request
  • May 2.2019             -  No forward motion, we called again and despite there was an officer assigned to this service request nothing happened/ no extension. We filed an InfoPass and have an appointment for Monday 6th for passport stamp.
  • May 6, 2019 - Attended InfoPass appointment in Philadelphia local USCIS local office for 930. This was for a never rcvd 797 for our 14yo dependent. Arrived 9:15, not crowded at all. No extra photos or fingerprint taken. We were in the car (lots of parking available) at 945. Pleasant female officer, she updated and checked for no changes. Stamped passport, explained is valid for 1 year. The took the green card which expired 5/4/19. No issues or problems.. I asked about the non receipt and she said it's pretty common.. 
  • August 23 - Biometrics NOA, scheduled for 9/3 @1pm
  • September 3 - Biometrics
  • September 4 -Fingerprint Review was completed
  • January 8 - ROC, Card Being Processed, Citizenship is next!
  • January 15 2020  - Cards Delivered

 

N400 Citizenship :)

 

  • November 28, 2020     - N400 Filed online and paid via CC
  • November 28, 2020     - NOA received electronically

I had my vaccinations transcribed a year and a half ago and back then yes, they did charge me to transcribe and complete whatever vaccinations I was missing, (had to do it for school back when I was under a student visa) so by the time I had to do medical exam for AOS I had all vaccines ready and up to date.

I do plan to go back to the same civil surgeon who did my AOS medical exam, (which by the way, I found him through the USCIS website on their "find a doctor" tool, it can't be just anyone) -_-
