Sorry if I missed something, but since it's 14 months(same for us), are you going to do a new medical exam? We've been thinking about finding a uscis doctor, but so far the reviews scare me, I'm not a fan of medicals in general.

 

We just sent out EAD/AP last week too, all in one envelope.

There is an automatic 180 days extension if your apply for EAD renewal before the expiration of your current card. 

You have to do another medical examination since the new forms have additional test (STD) that have been included.

You can try the free immunization centers for booster shots if you need them, that will reduce your examination costs, then go with your old medical exams and show them...You might be lucky to get cheap examinations.
