My fiancee never received her package from the US consulate. We filled out the form online to contact them and they emailed us a paper stated they received our petition from NVC. We went online to get all the paperwork. Even after her interview she still did not get any package.

I am worry if when her visa get approved. Will her passport with visa stamp get lost too?

My fiancé hasn't received anything in the regular mail either. I emailed them and they emailed me back packet 3 instruction letter, which is what we need for the medical exam. Medical exam and interview are both scheduled. Hopefully we won't have any problem getting the Visa once she passes interview.
