Is this the document cover sheet I received from the NVC that list all the documents needed or different like the IV Document Sheet. If it's not the same how can I get it when I paid my aos I didn't print out a document cover sheet I only printed the IV. Hope someone would be kind enough to answer. Thank you.

Yes it is the document cover sheet that you received from the. Nvc.

You will need that with either the bar code document sheet from the AOS or the Iv payment

Since you have the Iv bar code cover sheet you can use that one with the nvc document cover sheet

Send both the AOS and the IV packets together to the Address on the IV bar code document sheet

The only difference between the AOS and the Iv Barcode cover sheet is the departments

The address is the same for both. The NVC request that both packets be sent together so it does not matter which one you use.
