How did you get that many lines? Anyway, here are some things to fix, and a question haha:

Your most recent "self employed" could just say "PRESENT" instead of "TO PRESENT" ...just nit-picking.

Next, EVERY right column date should match the left column date of the row above it. If not, it is assumed there is a gap in time where you were unemployed. If that is the case, you have to list it.

Yours doesn't match when you went from self employed from 09 / 2012 until 07 / 2014, then changed to paint and body work on 02 / 2014 (that's 5 months backwards haha)

Also, for Fiber Glass Boat Repair, you have from and to as the same month.

Last, from leaving Benson Barges to your current self employed, you have a one month gap (10 / 2014 to 11 / 2014) Those should be the same month.

I got so many lines because I made my own table and was going to put it in a separate cover sleeve on the page behind the G-325a. Should that be okay?

Okay, so the reason it doesn't match is because I never left the Self Employed Job. (I was payed witha 1099 so it's self employed but I was working for a small business cleaning and doing maintenance work. I started in September 2012 and left in July 2014. Only worked one or two day's a week. And then in February 2014 I started working part time at the paint and body shop.

Does that make since or do you still think it should probably be put in a different order?

I put the Fiber glass boat repair that because it was a one week job I did on the side for someone in May of 2014. That should be fin, right?

Mmmm, maybe, I don't know. This whole deal confuses me. haha

I left benson barges around October 25th. I was then Unemployed for about 3 weeks, and started my current job on the 17th of November. Should I add a line where state that from october to november I was unemployed? Either way I think it might be confusing but I don't really know.

Sorry for such a late response!

First, having it behind the G-325A should be find, but on the indicate on the G-325A that you have the information behind it (silly, I know, but apparently it helps)

I didn't know you were working two different jobs (well, self-employed while working at another job) at the same time. Unfortunately, I don't know how to fill the form out if you have two jobs at once. Hopefully this response bumping the topic will allow for better answers from others.

Question for others regarding OP's post: How does one go about filling out the form when you work two jobs at the same time?
