Concern about Vaccination Supplement (Form DS-3025) for AOS

Timeline

Hello to everyone...You don't need vaccinations during the K1 process, because it is a noinmmigrant visa. But YES for Adjustment your status. You don't need your complete medical examination. You only need the DS-3025 signed for a civil surgeon from USCIS. You can find one in "find a doctor" in USCIS page. The doctor in Salem just charged me $20.

That would work, but neither one of us was given a copy of our DS-3025, so how could we have it completed by a civil surgeon?

Naturalization Timeline
02/08/2022 - N-400 submitted online

02/09/2022 - status update (your case is being actively reviewed by USCIS)

02/09/2022 - letter received in online inbox - biometrics being reused, no appointment required

08/01/2022 - interview notice received

09/20/2022 - interview scheduled

09/21/2022 - Applied for passport book & card and registered to vote

Timeline

You have to download and print the form in USCIS page. Just fill out with your details (No sign the form). The civil surgeon fill out the rest of the document. Bring with you the evidences of your vaccinations in case you have. He will decide which ones will be require to you, according your age and health.

Yes, and unfortunately it seems that different countries have different requirements in regard to the vaccinations during the K-1 process. I was informed that I needed to have mine completed before U.S. entry and if there had been any outstanding ones at the time of my medical, then I would have needed to get them done during my appointment.

I did want to say that I believe I found my DS-3025, after all that. So, I apologize for starting yet another thread on the topic. I honestly did not believe I had it, and was getting very concerned. What I have learned though, is that many people are not given this form, and I do think that definitely needs to change. Anything that is required should be provided without question.

I truly hope everyone struggling with this situation gets it sorted out.
