Submitting I-130 - previous abandonment of LPR by I-877/I-407

I abandoned my previous permanent residency on entering the USA in 2008. I have a copy of a form number I-877 with attached form I-407. Do I need to submit a copy with my husband's new I-130 application for me - the I-130 just says if you have previously been a permanent resident you will need to submit I-407, with a link to download the form. Does this mean they want a copy with the application? If so should I include the I-877 also? Thanks.

*facepalm* Just read the documents... even though the sworn statement clearly has my correct name, he's put the wrong name in all the official bits and it's absolutely littered with other mistakes! It's got the right A# at least! Think I may need to put a note of explanation in.
