What should I bring to my medical examination?

  • DS-260 Confirmation Form (strongly recommended).
  • Appointment letter sent by the National Visa Center or an e-mail confirmation from the Visa Information and Appointment Service.
  • Appropriate medical examination fee.
  • Passport (valid for at least 6 months)
  • Three color pictures (2 inches x 2 inches) with a white background(two are for the visa and one is for the medical report).

That is for the immigrant visa, K-1 requirement is slightly different.

What is this "Derivative 2 photocopies" some of this language dont even understand haist :crying:

Children.

Make sure to follow the requirement for K-1.

Hi just wanna ask?when is the right time of getting medical exam before interview?cuz some said 2-3 days,5-7days,2weeks,and 1month...cuz its very hard for me who stayed far from luzon.and going to manila all by myself and stay long in manila..:(

Embassy states 5-7 days. Follow the embassy instructions.

Applicants are advised to have their medical examinations done 5-7 working days prior their interview appointment at the U.S. Embassy in Manila; failure to complete the exam within the prescribed time frame may result in significant visa processing delays. Applicants are also strongly encouraged to register online with SLMCEC at their website: www.slec.ph prior to their visit. Registering online will save processing time and make the visit to SLMCEC faster and more efficient.

SLMCEC is located at 1177 J. Bocobo Street, Ermita, Manila. They can be reached by telephone at (632) 521-0020 and (632) 521-8647. SLMCEC is open from 6:00 a.m. to 4:00 p.m. It is best to arrive for your medical examination between 6:00 a.m. to 8:00 a.m. Applicants are served on a first come first served basis, but the order of release of the medical reports will depend on the complexity of the results of the medical examination.

What is the cost of the medical examination at SLMCEC?

  • The medical exam fee is US$245 for adults (15 years and older) andUS$185 for children (under 15 years of age).
  • SLMCEC does not accept credit card or dollar payments. The fees must be paid in Philippine pesos at the prevailing exchange rate.

DS-260? Is it a new form for K1?
