Hi, I have a question about my ds-3025. I had my medical back in february in London, I arrived into the states in May and then we got married the beginning of July. About to file AOS. During the medical I was told I was missing the 2nd MMR shot, they said to go to my doctor and get the 2nd one there and to get a note from my doctor to say I had it, which i did. Now I'm just looking at the ds-3025 and on the MMR section it says the date I had the first shot back in 1992, then in the "Vaccine given by panel site" column it says "k1". Then in the last column where all the other vaccines have a letter, the MMR box is blank. Do I need to see a civil surgeon to give me a new one with the updated MMR shot? thanks

How does your DS-3025 correlate with the great info that Nich-Nick has pinned in the thread at the top of the forum page?

How does your DS-3025 correlate with the great info that Nich-Nick has pinned in the thread at the top of the forum page?

It's the same, apart from in the last Column there is no code/letter corresponding to MMR. all the others have a letter in. Also in section 2 nothing is ticked, however I've read that the doctors in London do not tick anything in that section. Thanks

Yes you need an MMR and proof (which you got) and everything put on a new form (I-693) and signed off by a civil surgeon. Your DS-3025 is incomplete.
