Where to place Form G-1145?

A. I am assembling my N400 application. Do I place my G-1145 form before or after the N-400?

B. Also, please let me know if the following order is correct:

(1) Check for $680

(2) Photos

(3) N-400

(4) GC front & back photocopy

(5) Additional sheets (for travel history and organizations associated with)

C. Also, I am not stapling anything, I am using those big black plastic clamp-like clips. I also plan to use a standard flat-rate Priority Mail envelope from USPS to mail it out. Is this OK?

Pitaya the guide you refer to says absolutely NOTHING about Form G-1145. That was totally unhelpful...

Attach it on top of the application. Attach on top of the application 2 pictures and then the check

This how I did it;

1- 2 pictures

2- check

3- g1145

4- n400

5- GC photocopy

6- other supporting document.

If it fits in flat rate envelope, that's fine

day 1 -- 04/11/2012-- package sent to Chicago

day 2 -- 04/12/2012-- package was received.

day 43-- 05/23/2012-- Notice for an interview is received for 06/26 @ 2pm

day 63-- 06/12/2012-- Received a Text & email for an update- Card production EAD/AP

day 77-- 06/26/2012-- interview / approved on the spot.

day 86-- 07/05/2012-- Received my GC in the mail.

ROC

day 1 -- 04/07/2014 -- ROC Package delivered to VSC

day 16 -- 04/23/2014 -- Walk-in Bio.

day 197 -- 10/20/2014-- Approval Letter received dated 10/16/2014

day 202 -- 10/25/2014-- GC received
