middle name(s)??

Hi all

I am a DV 2016 selectee.there were three fields for name(s) in the original entry. In the family name, I entered my fourth name and in the first name I entered my first name but in the middle name I entered just my second name so I didn't enter my third name which appear in passport

Any help please

As Musfata said, use both, at the end the name on the visa will be same as the passport,

but in my case I have five names in my passport, the CO put

Last name: Fifth

First Name: First

Middle names: Second and Third

he did not put the fourth name.

My husband uses his second name as his last name, so we put the first name in the first name slot and all the other names as his last name. We put nothing in the middle name. His EAD/AP card now prints it with all his names--one first name and 4 last names.

6.14.2018   Interview Notice sent

7.24.2018   Naturalization Interview; approved

9.26.2018   Oath Ceremony Scheduled
