Renewing EAD/AP I-94 Not Up-to-date in government's system

As we are renewing my husband's EAD/AP combination card, I noticed people are including the most recent I 94 form. I figured since he left once during the application process, I should try to print a new I-94. When I went to do this, the information provided was that he left the country through Atlanta on September 5, 2014, but it doesn't show his return on November 14, 2014. Can we just print his passport stamp and send it in or do we need to try and fix the I-94?

for the I-765 renewal, we only provided the I-94 for POE on the K-1 visa....for the I-131 renewal we did provide both the POE I-94 and I-94 travel history, not sure if travel history is necessary as we have read here that subsequent trips taken after original POE on the K-1 visa are not really considered as entries, but paroles. At the I-94 site i believe you can choose last re-entry or travel history, maybe his parole back into U.S. shows up on one but not the other.

We did go to the I-94 site, and the travel history only shows he left...not that he returned. In the passport is a paroled paper that is stapled by the K-1 visa. The officer wrote an admission number with a paroled stamp. I just don't know where to go to type in the admission number to get that history. He has 4 last names, and we've had troubles before with his original I-94 because of mistyping his name. We think that once he entered the country, this issue should have been resolved for his reentry. Could we just make a copy of the paroled paper in the passport and attach it with the original I-94? Or, does anyone have a web address where you go to type in your admission number to get the information we are seeking? I can't type it on the I-94 site.
