Form I-751 ( Petition to Remove Conditions on Residence ) Question

In Part 1: Other names used:

1. I have to include include my Previous Married name ( that got annulled) , right?

2. Also, there were other names that were mistakenly entered in our health insurance from before. Reason was because, our insurance is from the Military, and when I got my first Military ID card they placed my last name as a hyphenated last name which was XXXXX Middle-Last. Example..Instead of MARIA LOPEZ REYES-CRUZ ( which was how the SS Card showed ) they Placed MARIA LOPEZ-CRUZ. I didn't realize the mistake until I had my first appointment with my PCM.

3. Not sure why too, but our lease just showed also MARIA CRUZ. My SS# is on the lease tho ( the last 4 digit - they got my SS Card as I remember and did a background check but still I am not sure why they didn't use the name in my SS Card )

Do I have to write this 2 names too ( #2 & 3 above ) ?

1. Yes. If this was once your legal name, you have to include it.

2. No. This was a clerical error and NOT your legal name. You should not include this one although you should include a note stating that a clerical error was made if you are submitting the health insurance showing the erroneous name.

3. No. Usually American leases show just 2 names. Your first e.g. Maria and your last e.g. Cruz, which I assume is your husband's last name and they assumed was yours as well. Again, an error does not make it your legal name.

Good luck! :)
