Marriage Registration Form

Hello guys!

We're in the process of getting the form ready to obtain our marriage license.

I am a little confused as what to put in the field for father/parent name prior to first marriage:

My mother re-married and I've been living with my stepfather. However, my last name has not changed -- I still have the last name as my biological father. So on the form, would I put down my stepfather or my biological father's name?

Any help would be appreciated.

Also, another question...I'm away in grad student in another state, should I put that state as my "residence" or the state that I am from?
