I'm just about burned out for the day trying to gather all this info, reading these forms, checking off what they got so far, making phone calls to civil surgeons, etc. My mind is already muddled and can't think straight. Need help translating into plain english what these two vaccination records say, and what it means in regards to AOS.

Any help would be appreciated. Much mahalos!

So far, I've come to the conclusion that figure A will need:

  • TDap
  • Measles update
  • Mumps update
  • OPV/IPV update
  • Help B
  • Varicella (update?)

  • TDap (update?)

Just want to verify if I'm correct so far. I'm very tight on money right now being sole provider and want to minimize Dr visits.

Make sure to also check with the school your k2 would be going to if he/she is school age. Our K2 is playing catch up, even though they followed federal guidelines!

Ahh, thanks for the reminder! My K2 is 9yrs old. I want him to start here at the appropriate grade, but his mom and my family think he should start one grade below. 4 against 1, so he's gonna start one grade below lol. New move, new country, new language, etc. Too overwhelming, they say.

From figure A your child may be required to receive an HEP B vaccination. A HEP B vaccination is required from birth until the age of 18, and the DS-3025 does not give any indication of that vaccination being received. The other vaccinations you mentioned (the updates) are not a requirement for AOS as only the first shot in a series is required.

From figure B, it appears you have received all required vaccinations. I'm not sure why it was marked on the form "Applicant will request an individual waiver based on religious or moral convictions" because it looks like all the vaccinations required are covered.

In short, your child should only need the HEP B vaccination, and you should not need any.

For reference, consult this table from the CDC regarding AOS vaccination requirements: http://www.cdc.gov/immigrantrefugeehealth/exams/ti/civil/vaccination-civil-technical-instructions.html#tbl1

Thank you! I actually printed that CDC table and did side by checkmark kind of thing for both records. Just I have no one in person I would trust to verify my thoughts. I was also not sure why that "religious or moral reasons" was checked off.

Does figure A need TD or Tdap?

Do both need TB clearance?
