Is my DS-3025 filled in correctly?

Hello fellow VJ'ers,

I was wondering if my DS-3025 has been filled out correctly for AoS (see attachment below).

My doctor ticked and dated MMR but did not tick the column "Insufficient Time Interval" or any other column to the right of it. My medical exam was done on July 31 and the MMR/tdap vaccine was given on July 25. However, down the bottom he ticked "Vaccine History Incomplete - Applicant may be eligible for blank waiver(s) because vaccination(s) not medically appropriate (as Indicated Above)." Is this correct or will I have to go to a civil surgeon and fill out form I-693?

Where in Aus did you get this done? They were supposed to date DTaP too because it is needed and it is age appropriate. You'll need to see a family doctor here in the states to get DTap and get them to transcribe the vaccinations on your DS-3025 onto an I-693 form (you won't need a physical exam again, just vacinations transcribed with DTap added).

From USCIS http://www.uscis.gov...on-requirements: Under the immigration laws of the United States, a foreign national who applies for an immigrant visa abroad, or who seeks to adjust status to a permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases:

  • Mumps
  • Measles
  • Rubella
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Haemophilus influenzae type B
  • Pertussis

05-18-2022: Filed N-400 online. Received online NOA and Biometrics re-use.

06-03-2022: Interview scheduled (online notice).

06-10-2022: Interview letter received via USPS.

07-11-2022: Naturalization Interview

Where in Aus did you get this done? They were supposed to date DTaP too because it is needed and it is age appropriate. You'll need to see a family doctor here in the states to get DTap and get them to transcribe the vaccinations on your DS-3025 onto an I-693 form (you won't need a physical exam again, just vacinations transcribed with DTap added).
