The tax transcripts would be sent to the NVC with the AOS package and the embassy would have that there on the day of the interview. I don't know how much the medicals cost in Ghana.

Timeline

If you are going via K1, then Ghana Embassy doesn't require tax transcript. I advise you do medicals at Akai Clinic which is about 2mins walk from the embassy. The cost wouldn't be more than GHc 1,000.

Yes you will need your taxes because it's a spouse visa, this is sent to the NVC as part of your AOS package.
