Hi, back in my home country had my medicals twice, cuz the first time medicals expired and had to re do them just now on june, twice them gave me just a sealed disc, and no ds 3025 , and they told me results was good and will be sent to the embassy.

Now after people telling me send even ur i 693 form with ur aos package , i thought ok let s give it a try, and went to the civil surgeon, he checks out the disc and says there is nothing on here ecxept the chest x-ray, and there was no ds 3025 on there.

He told me that it would cost me now about 489 dollars for everything, 5 shots including the flue shot cuz it s flue season and plus urine analysis and physicals, ect.

Me and my wife are so stressed out and don t know what to do, we have no money right now to re do the thing, we really need some help , some advice, should i just send the AOS package without the i 693 form since i had them twice on my country ,please give us some ideas.

Here is a good thread that speaks about Form I-693 and as it relates to the AOS process, posting #5 has very good information: http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/
