medical for K1 / DS-3025 / I 693

Timeline

OK My fiancée had her medical examination in Juarez, MX. She was given a seal document which the person at the interview kept. The Only thing she has is a CD disk and a ONE PAGE DS-3025 (FOR SOME REASON SHE DID NOT GET THE SECOND PAGE (I think is the instructions page, not sure)).

SHE HAD HER MEDICAL DONE APPROXIMATELY JUNE 9, 2014. And there is where she was given the ds 3025. to make things worse the ds 3025 that she was given has the following in the corner:

OMB No. 1405-0113 Expiration Date: 08/31/2014 Estimated burden: 30 minutes (see page 2 of 2). [again we have no page 2]. THAT MEANS THEY GAVE US A FORM THAT HAD A QUICK EXPIRATION DATE AND HER 90 DAYS FOR MARRIED ARE NOT UP YET [WE ARE GETTING MARRIED IN LAS VEGAS NEXT WEEKEND]. tHE ds 3025 IS SIGNED BY A DOCTOR BUT OTHERWISE IS BLANK.

1) From my reading I do not have to submitted a full I 693, just the front page of a I 693. But wouldn't that look not fluid, it simply does not make sense how you read the I-693 that a doctor would sign off only the front page and DS 3025. Any thoughts??

2) Will fiancée be ok with submitting a new DS 3025 (since that form expired) with new doctor's signature?

3) Is having the DS 3025 filled out the same thing as vaccination transcription???????

I heard people get RFE for not having vaccination transcription, but isn't that the same thing as having DS-3025 filled out??

4) Is it best to include a full I 693 with DS 3025 so I won't get RFE?

5) Do you guys know of any Civil Surgeon in Los Angeles are that will do the DS 3025 without I 693 and that is not expensive on the vaccinations?

You do not have to sumbit a complete I-693, however, because DS 3025 is blank, you will have to do the front page and the vaccinations page complete by a civil surgeon.

DS 3025 is the vaccination transcript, the reason why it was given blank its because at the medical exam in Juarez, they did not ask for vaccination records nor did they give your fiancee any vaccinations because for a K1 they are not required, however, they are required for AOS process.

4) Is it best to include a full I 693 with DS 3025 so I won't get RFE? That depends on you, sometimes the civil surgeon, there is times where they only do the whole I-693, which is a series of questions and vaccinations.

5) Do you guys know of any Civil Surgeon in Los Angeles are that will do the DS 3025 without I 693 and that is not expensive on the vaccinations? You can go to the USCIS website and use the civil surgeon locator, you can call and ask what the prices are. They will not re-do the DS3025, you will have to submit the copy your fiancee was given the it is with the I-693

I'm having a similar problem with my husband's vaccines. To help you out, you can look for a civil surgeon near you by looking at this website (I live in the LA area also): https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

You can call and ask the clinic about their procedures. It will show you a lot of clinics. :)

Thanks guys,

So I can go ahead it do the ds 3025 with the form that was given. Even if the form has an expired date?? I have seen the I 692 front page, it seems like they just can't incorporate the ds 3025, it flows like they have to do the whole exam. Hope I explained myself correctly.

Don't do anything with the DS3025, leave it like it is. However, because vaccinations are not annotated, you have to do the first page and vaccinations page of the I-693. But depending on the civil surgeon, they might need you to do the whole I-693. You will include both the DS3025 (blank) and the I-693 which will be given to you in a sealed envelope by the civil surgeon
