After been married to my US wife and we want to submit our application for the green card now she's having my surname on her state ID but her birth certificate as her formal surname do we need to get an affidavit for name change or the marriage license covers her while submitting my document along with my forms.?

She doesn't have to show any affidavit. Her marriage certificate is fine.

If she has received her passport within the last year she can get it changed to her married name free. Over a year old she will need to pay. Also she should take her Marriage certificate to Social Security office and get that changed.
