They closed the hospital--how do I obtain records *Ontario*

I had a mental health crisis back in 2007/2008 that resulted in a hospitalization. Ontario medical records had not yet been centralized and I did not have a family doctor for them to send the records on to, at the time. I know I'll need records from this in order to complete my medical.

The hospital has been closed and has been replaced, in St. Catharines. This apparently occurred last year. I had not lived there or been treated there since early 2008. Does anyone know what happens to health records when hospitals close in Ontario? Or can I just take in an affidavit from my most recent therapist or something? I've got a few months to figure this out, but I want to figure it out sooner rather than later.

2011: Realized we needed to evaluate our status as friends when we realized we were talking about raising children together.

2011/2012: Decided we were a couple sometime in, but no possibility of being together due to being same sex couple.

June 26, 2013: DOMA overturned. American married couples ALL have the same federal rights at last! We can be a family!

June-September, 2013: Discussion about being together begins.

November 13, 2013: Meet in person to see if this could work. It's perfect. We plan to elope to Boston, MA.

March 13, 2014 Married!

May 9, 2014: Petition mailed to USCIS

May 12, 2014: NOA1.
October 27, 2014: NOA2. (5 months, 2 weeks, 1 day after NOA1)
October 31, 2014: USCIS ships file to NVC (five days after NOA2) Happy Halloween for us!

November 18, 2014: NVC receives our case (22 days after NOA2)

December 17, 2014: NVC generates case number (50 days after NOA2)

December 19, 2014: Receive AOS bill, DS-261. Submit DS-261 (52 days after NOA2)

December 20, 2014: Pay AOS Fee

January 7, 2015: Receive, pay IV Fee

January 10, 2015: Complete DS-260

January 11, 2015: Send AOS package and Civil Documents
March 23, 2015: Case Complete at NVC. (70 days from when they received docs to CC)

May 6, 2015: Interview at Montréal APPROVED!

May 11, 2015: Visa in hand! One year less one day from NOA1.

They should retain the records at the new hospital. The St. Catharine's hospital re-located but the records should have gone with them. Records are mandated by law to be kept for 7 years (I believe). Check out this page http://www.niagarahealth.on.ca/en/health-records there is a form you can submit to have your records released to you. You can also call the the release of information department at 905-378-4647 x4470 M-F 0800-1600 and maybe they can help direct you further.

Thank you SO much. That's a huge help. I was sort of going 'gaaaah how much calling am I going to have to do????' I'm out of the country right now. Let's hope that they still have my records--It's been just over seven years.

If the records no longer exist, then bring the letter you receive back from the hospital advising of that and take it with you to your medical. If you get that information over the phone, then ask them if they can provide a written statement of that fact. That way, if you are asked, you can show that you have tried to obtain the documentation but it no longer exists. (I had to do that for a court case - also in St. Catharines, interestingly enough- for which the records no longer existed.)

A statement from your current therapist stating any medication you are on and your current status would also be appropriate.
