1) At what point is the immunization record verified - is it during the medical interview prior to the interview itself? Or is it done before this? And is it done by any doctor?

2) What form is needed for the police background report? Can it be done by any police station or does it have to be linked to the embassy in the fiance's home country?

Hi,

Immunization record is asked by the doctor when you visit for medical. If you do not have immunization record, doctor prescribes few depending on age and condition.

For police verification, you will need to produce Police Clearance Certificate called PCC from regional passport office. There are lot of posts regarding PCC. Search and read them.
